No student at Glen Crest Middle School shall be permitted to compete in a try-out, practice, or game for interscholastic sports unless the student has filed with the school health office a Pre-Participation Form issued by a licensed physician, physician’s assistant, or nurse practitioner not more than 395 days preceding such tryout, practice or contest in any athletic activity.

You may submit documentation on the Certificate of Child Health Examination or a Pre-Participation Form to the building nurse.
