May 18 COVID-19 vaccine clinic in Glen Ellyn is open to anyone 12 or older
Anyone 12 or older can now register to receive the first dose of the Pfizer-BioNTech COVID-19 vaccine at a clinic that will be held Tuesday, May 18, in Glen Ellyn.

The clinic will offer first-dose vaccinations on Tuesday, May 18, with second-dose appointments on Wednesday, June 9. This week, the CDC approved the Pfizer-BioNTech COVID-19 vaccine for 12- to 15-year-olds.
Appointments for May 18 are available from 8 a.m. to 3:45 p.m. at the Ackerman Sports and Fitness Center, 800 St. Charles Road, Glen Ellyn. Anyone who is vaccinated on May 18 will automatically be scheduled for the second shot at the same time and location on June 9. Families are asked to only sign up for May 18 if they can also get the second shot on June 9.
There is no cost to be vaccinated. Children between 12 and 17 years old must be accompanied by an adult and have a signed consent form. The consent form (Espanol) will be emailed to you once you register. Please print the form and complete it before arriving at the clinic.
Students who need to step away from in-person or remote learning to be vaccinated will have an excused absence. The vaccine is not required for CCSD 89 students. The appointment will take about 45 minutes.
Anyone 18 years or older who is being vaccinated will need to bring proof of identification, such as a photo ID. Although the vaccination is free, anyone who has a current health insurance card should bring a copy of it.
